Before we get into sex after surgery, let’s talk about the different types of MTF bottom surgeries and how they affect sexual intimacy.

Full-Depth Vaginoplasty

Full-depth vaginoplasty is the most common gender-confirming surgery for trans women. This procedure creates a neovagina with depth and allows for penetrative sex. During the surgery, penile and scrotal skin is used to construct the vaginal canal, and in some cases, additional tissue grafts from other parts of the body are used to achieve the desired depth.

The clitoris is created from the glans of the penis and repositioned for erogenous sensation, and the labia are formed from the remaining penile and scrotal tissue. The result is a functional and aesthetically pleasing vagina that, with proper care and dilation, can accommodate vaginal penetration.

Minimal-Depth or Zero-Depth Vaginoplasty (Vulvoplasty)

Minimal or zero-depth vaginoplasty, also known as vulvoplasty, provides external genitalia that looks like a natural vulva, but no vaginal canal is created. This option is for trans women who do not desire penetrative vaginal sex or prefer not to undergo more invasive full-depth surgery.

While vulvoplasty gives a feminine appearance and often retains erogenous sensation, it does not allow for full vaginal intercourse. However, many patients report that satisfying sex is still possible for those who choose to undergo this procedure.

Remember that orgasm is highly emotional and is possible with minimal or zero-depth vaginoplasty, as you will retain all of the nerves necessary for stimulation, even without a vaginal canal.

For those patients who do not need or want vaginal penetration, this procedure offers much less surgical risk, fewer complications with healing, and an overall excellent cosmetic appearance. Because of the avoidance of some of the minor complications inherent in full-depth vaginoplasty, this option often has better cosmetic outcomes. This option also removes many concerns associated with the need for regular dilation.

What Is the Timeline for Sex After Full-Depth Vaginoplasty?

The timeline for sex and penetrative intercourse varies from person to person and depends on their healing process and post-op care. Here are some general guidelines to be aware of:

First Few Weeks Post-Op

In the first few weeks after vaginoplasty, the focus is on healing and preventing complications. It’s important to remember that swelling, bruising, and discomfort are normal, and patients are advised not to have any form of sex during this time. The new vaginal canal is delicate and needs time to heal properly, so having patience is definitely helpful.

Dilation

For trans women who have had full-depth vaginoplasty, dilation is a crucial part of post-op care. Dilation involves inserting medical dilators into the neovagina to maintain its depth and width while preventing the vaginal canal from narrowing or closing. Initially, dilation is done multiple times daily and gradually reduces in frequency over the next few months.

Dilation is not only for maintaining the structure of the neovagina but also for preparing it for penetrative sex. As the vaginal canal heals and gets used to dilation, it will gradually become more comfortable for sex.

Vaginal Intercourse

Most surgeons recommend waiting at least three to six months after vaginoplasty before having penetrative sex, but this can vary depending on your individual healing experience. Consult with your surgeon for personalized advice.

When you do have vaginal intercourse, it should be done with care. Use lots of lubrication, be gentle, and go slow, as the neovagina is still sensitive. Some discomfort is normal during the first few times but should improve over time as the tissues heal and adapt.
